Anemia is one of the most common pathological conditions encountered in primary care. Its consequences, although mild in most cases, are potentially very severe. It can be detected by screening and, in most instances, treated readily and cheaply.

It may serve as a warning of various potentially fatal disorders and, depending on the underlying cause, of disease processes leading to irreversible changes in vital organs and tissues. All nurses should be aware of the nature of anaemia and how it can be detected. This includes having a knowledge of the following:

  • Its signs and symptoms and the types of laboratory diagnosis available.
  • The screening and preventive measures available.
  • When a diagnosis of anaemia should be a trigger for investigation of other potentially serious diagnoses.

Anaemia occurs as a consequence either of deficiencies in the production of mature circulating red blood cells (RBCs) or of excessive loss or destruction of these cells. All forms of anaemia can be classified on the basis of the balance between these two processes.

Alternatively, classification can be determined on the basis of the morphology of the RBCs, which can give valuable indications of the underlying cause.

It is important to stress that anaemia is not a disease, but a pathological state consequent upon an underlying disease; for example, nutritional deficiency (iron deficiency), inherited abnormality of haemoglobin synthesis (sickle cell disease, thalassaemia), or haemorrhage (colon cancer).

The impact that anaemia may have on a person's quality of life and general health is considerable, especially for older people. A recent report shows that the condition can contribute to a general decline in the physical performance of older people, with loss of capacity for independent living. The World Health Organization has estimated that, globally, at any one time, 2,088 billion people have anaemia (UNICEF et al, 2001). Indicates the prevalence of anaemia in at-risk groups.

Haematinics are the chemical agents or substances which are required for normal erythropoiesis.
These are—iron (Fe), Cobalt (Co), Zinc (Zn), Vit-B12, Folic acid and Erythropoietin.

Iron (Fe):
Required for Hgb production. In the absence of adequate iron a small red cell with insufficient Hgb will be formed giving rise to microcytic and hypochromic anaemia. Fe** forms the nucleus of the porphyrin ring, which when combined with appropriate globin chain forms Haemoglobin. Intake of Iron is 20mg/day from which 1mg/day (5%) is absorbed. Iron is normally available in the diet in the form of haem or iron complex to various organic compounds.

Absorption of iron:
Iron is most readily absorbed in the ferrous state. But most of the dietary iron is in the ferric form. No more than a trace amount of iron is absorbed in the stomach. But the gastric secretion (HCl) dissolves the iron and permits it to form soluble complexes with ascorbic acid (vit-C). Vitamin-C and other substances aid its reduction into ferrous form. Most of the iron is absorbed in the upper part of the small intestine that is duodenum and upper part of the jejunum. Shematnic contains Ayurvedic drugs which are rich in dietary iron like Draksha, Bhringaraj, Sariva, Vidang. Amlaki contains vit c, which aids in the absorption of iron.

Loha bhasma is a calcinized iron. Prepared using Shuddha Loha, Shuddha Hingu, Kumari Rasa & Triphala Kwath. Loha Bhasma is excellent Haematinic and Balya. Thus, it increases quality, quantity, and strength of Rasaraktadi dhatu.
It is the choicest remedy for anemia. As it is astringent, it is useful to arrest bleeding, and hence is salutary in menorrhagia and leucorrhoea.It works well in anemia. Loha bhasma is one of the best rejuvinative, as it replenishes the blood, it nourishes all tissues. It is recommended in anemia, hepatosplenomegaly, skin diseases especially of pittaja type, obesity, loss of memory and weak eyesight, jaundice, oedema, piles and as a general tonic.
Maricha and Pippali also contain dietary iron, and play a pivotal role as bio-enhancers.
